GENERAL

Thank you for observing our “Smoke Free Campus” policy.

To each and every visit please bring your current insurance card and your medications including over the counter medications and vitamins. You will be asked to verify your current address and phone numbers. If you are a new patient, you will be asked for a photo identification.

APPOINTMENTS

Appointments may be scheduled for clinical visits and for lab visits by calling (334) 566-9800. Please notify us 24 hours in advance if you cannot keep your appointment, including appointments for labs. These appointments may need to be rescheduled. Patients who fail to keep a scheduled appointment may be subject to a service fee. Patients who arrive 15 minutes late to their scheduled appointment will be worked in if possible, or rescheduled.

INSURANCE

Many insurances now have deductibles. Please review your deductible requirements before your visit as full payment for the visit may be required at the time of service until the deductible is met. Please bring your current insurance to each and every visit.

ALL charges are the responsibility of the patient. As a courtesy to our patients, we file for you for most major insurances and Medicare. To avoid unexpected charges, please address insurance questions to the staff prior to your visit. Co- pays are collected at check in. Please present your valid insurance card at each clinic visit. We accept Visa, Mastercard, cash and checks.

Many insurance groups restrict “same day services”, that is, they do not permit an office visit charge on the same day as a procedure or test charge. We appreciate your understanding in this matter when you are scheduled for a return visit for these tests and procedures.

PRESCRIPTIONS

Allow 2 business days for your prescription to be processed and called to your pharmacist. Contact your pharmacist for the status of your refill before calling our office.

Prescriptions prescribed by another physician need to be refilled by the ordering physician. Please contact or have your pharmacist contact the ordering physician.

Narcotics are not prescribed nor refilled after hours.

PATIENT PORTAL

We now offer a secure portal access that allows the individual patient to request renewal prescriptions, request appointments, and view some of the patient record such as labs. For more information and /or to sign up for this valuable service please contact us at the office or sign up when you are in for your next visit.
